Horizon Construction & Asset Management Partners was co-founded in 2024 by Elliot Schreiber and Isaac "Yitz" Horowitz — bringing together deep healthcare operations expertise and a nationally proven real estate track record to serve owners, operators, and investors in multifamily housing and long-term care.

Horizon wasn't founded by general contractors looking for a niche. It was founded by operators who have spent more than a decade acquiring, renovating, repositioning, and refinancing real assets — and who understand that construction execution is inseparable from asset strategy.

Our principals have directed acquisitions, ground-up development, institutional repositioning, historic preservation, and mixed-use waterfront redevelopment. They've managed over 1,000 multifamily units, repositioned an 816,000 SF industrial campus, and led pre-development for a 38,000 SF waterfront mixed-use project. Every project was hands-on: from underwriting and entitlement through construction oversight, lease-up, and refinancing.

Elliot Schreiber brings deep expertise at the intersection of healthcare operations and real estate. As Chief Revenue Officer of 100 Boulevard Management — the holding entity for Senior Planning Services, Senior Community Trust, Senior Planning Community Solutions, and Census Track — he has spent years navigating the Medicaid, home care, and long-term care landscape. His background in residential and commercial real estate, including co-ownership and management of portfolios across New York, South Florida, and Central Connecticut, informs his asset-management approach at Horizon. Elliot's positioning across both the operational and capital sides of healthcare and real estate makes him an exceptional resource for owner-operators and investment managers alike.

Isaac "Yitz" Horowitz
Co-Founder

A full-cycle real estate operator and developer with more than a decade of hands-on experience across multiple asset classes. As founder of Hudson Communities, Yitz has built a 1,000+ unit multifamily portfolio across New York State through disciplined acquisition, hands-on renovation, and active asset management. He led the full repositioning of an 816,000 SF industrial campus in Maryland — including a $5M state-funded historic preservation outcome — and directed pre-development for a 38,000 SF mixed-use waterfront project in the Hudson Valley. Early in his career, Yitz worked for E&M Management, one of the largest owner-operators and developers in New York City. He went on to hold a senior role at Rhodium Capital Advisors, a nationally recognized owner-operator where he oversaw a multifamily portfolio spanning markets across the entire country.
