His background in residential and commercial real estate, including co-ownership and management of portfolios across New York, South Florida, and Central Connecticut, informs his asset-management approach at Horizon. Elliot's positioning across both the operational and capital sides of healthcare and real estate makes him an exceptional resource for owner-operators and investment managers alike.

Co-Founder

Isaac "Yitz" Horowitz is a seasoned real estate operator with a distinguished track record in residential and commercial property at a national scale. As founder of Hudson Communities and Rapid Improvements, he has built and operated substantial real estate portfolios with a focus on value creation and operational excellence. Early in his career, Yitz worked for E&M Management, one of the largest owner-operators and developers in New York City, where he developed a foundation in large-scale construction and asset management. He went on to hold a senior role at Rhodium Capital Advisors, a nationally recognized owner-operator where he oversaw a multifamily apartment portfolio spanning markets across the entire country. At Horizon, Yitz leads construction operations and capital improvement strategy — bringing the discipline and execution rigor of institutional-grade, coast-to-coast real estate experience to both multifamily and long-term care.
